Capitation (CAP) is a specific dollar amount that was established to cover the cost of healthcare services delivered to an individual. CAP usually refers to a negotiated per capita rate to be paid, usually on a monthly basis, to a health care provider. The provider is responsible for delivering or arranging for the delivery of all health services that are required by the covered individual under the conditions of the provider contract.
